Responsibilities Responsible for the full delivery of population interventions, including assessment, individualized wellness strategies, health enhancement and health maintenance strategies. Perform patient wellness tests, clinical assessments and evaluations. Design, execute, and evaluate safe, effective wellness programs for individuals and groups. Apply current knowledge, theory, evidence and sound clinical judgment in patient management. Review, analyze, and document patient results in clinical documentation tools. Assist with department and program planning. Research best practices, measure effectiveness and performance. Participate as member of collaborative research teams that examine relationships among nutrition, physical activity, resiliency and overall health. Understand and promote the Healthy Living Program philosophy and assist patients with establishing an ongoing relationship with Mayo Clinic. Qualifications Licensure or eligible for licensure in the state of Minnesota. Advanced degree in a health‑care field. Board certification in clinical specialty area. Active participation within professional organization. Demonstrated coaching skills including reflective listening and motivational interviewing. Excellent customer service skills to foster a premium patient experience. Physical therapy licensure in the state of Minnesota. Limited Scope X‑ray Operator certification through the Minnesota Department of Health within six months from hire date. Basic Life Support certification. Advanced skills in organization, communication, interpersonal relations, leadership, decision making and problem solving. Compensation $68,373.76 – $102,602.24 / year Schedule Full time; Monday, Wednesday, Thursday, and Friday; 64 hours per week.
