Outpatient Primary Care RN - PACT Team Leader

Diversity Group is seeking an experienced Registered Nurse for the Patient Aligned Care Team (PACT) in a primary care setting. As an RN, you will provide and supervise direct patient care, administer medications, and coordinate care across the care continuum. You will collaborate with an interdisciplinary team, lead education and training, and participate in quality improvement initiatives to deliver patient-centered outcomes. A valid nursing license and English proficiency are required. #J-18808-Ljbffr Diversity Group
