WHAT YOU'LL DO

Working under the direction of a Physical Therapist, you'll deliver skilled interventions that help residents move with greater safety, confidence, and independence.

You'll:

• Provide skilled physical therapy interventions in accordance with the established plan of care.

• Help residents improve strength, balance, gait, transfers, endurance, mobility, and functional performance.

• Use meaningful, functional activities to help residents achieve their goals.

• Incorporate the resident's actual environment and daily routines into treatment whenever appropriate.

• Recognize changes in mobility or functional status and communicate them promptly to the supervising PT and interdisciplinary team.

• Help residents work toward the things they actually want to do—walking to meals, participating in activities, navigating their apartment, or staying active in their community.

• Provide education and reinforcement to residents, caregivers, and community staff.

• Collaborate with PTs, OTs, nursing, caregivers, families, and community leadership.

• Build strong relationships with residents and families.

• Document efficiently and thoughtfully using Prava's technology and AI-enabled tools.

• Participate in a therapy culture focused on clinical excellence, teamwork, accountability, and resident experience.

• Help create a therapy presence that is visible and integrated throughout the community.
