Hospice Registered Nurse (RN) - Ocean County, NJ

Swan Hospice is seeking a compassionate and experienced Hospice Registered Nurse (RN) to join our team of dedicated caregivers. As a Hospice RN with Swan Hospice, you will play a vital role in providing supportive, holistic care to terminally ill adult patients and their families. Our team is committed to delivering personalized end-of-life care plans that are implemented with dedication, skill, and sensitivity.

Provide high-quality, patient-centered care to terminally ill adult patients, focusing on comfort, dignity, and quality of life

Collaborate with interdisciplinary teams to develop and implement individualized care plans, addressing physical, emotional, and spiritual needs

Conduct thorough assessments, develop and implement patient care plans, and coordinate care with other healthcare professionals

Educate patients, families, and caregivers on disease management, symptom control, and end-of-life care

Provide emotional support and counseling to patients and their loved ones

Document patient care activities, assessments, and outcomes in accordance with company policies and regulations

Current licensure as a Registered Nurse (RN) in the state of New Jersey

Minimum 1+ year of hospice experience (or related experience)

Strong knowledge of hospice care philosophy and regulations

Excellent communication and interpersonal skills

Ability to work independently and as part of a team

Compassionate and empathetic approach to patient care
