LPN – Private Duty | Night Nurse | 12 Hour Day Shift | IDD Community Care $35 per hour 12 Hour Shift – 10:00 PM to 6:00 AM Boyertown, PA 19152 1:1 Care Team Environment Make a Meaningful Impact in the IDD Community Isoke Developmental Services is seeking a compassionate Licensed Practical Nurse (LPN) to support an adult individual receiving In-Home and Community Support (IHCS) services. This position supports one adult individual in a structured home and community setting, providing skilled nursing support and daily assistance as part of a 1:1 care team. The role offers a stable daytime schedule and the opportunity to build a meaningful relationship while helping the individual maintain health, independence, and community engagement. What You'll Do Provide nursing oversight and health monitoring in accordance with the Individual Support Plan (ISP) Administer medications and support G-Tube feeding and health protocols Monitor vital signs and report health or behavioral changes, seizure monitoring, respiratory treatments, skin checks, repositioning, incontinence care, and ongoing observation of condition.
