Description: This position involves managing patient care from admission through discharge. The Case Manager will work collaboratively with a multidisciplinary team to ensure optimal patient outcomes. The role requires strong communication skills and the ability to assess patient needs. Weekend shifts are required every other weekend. Candidates should be willing to float between similar units based on departmental needs. Experience with electronic health record systems is essential. Support for housing may be available for those in need. This opportunity is ideal for those who enjoy nature and outdoor activities.
