Charge Nurse - Registered Nurse (RN) - Long Term Care

Job Summary

We are seeking a dedicated and experienced Charge Nurse (RN) to join our Long Term Care team at Fall River Health Services in Hot Springs, South Dakota. This pivotal role involves providing comprehensive general nursing care to residents within a long-term care setting, ensuring adherence to individualized care plans, facility policies, and professional standards of practice.

The successful candidate will be instrumental in assessing resident health status, developing and implementing nursing care strategies, and evaluating patient responses to interventions. This role requires a commitment to promoting resident rights and choices, fostering a safe and clean environment, and participating in quality management initiatives.

As a Charge Nurse, you will serve as the primary contact person on your assigned shift and directly supervise Nursing Assistants/Resident Assistants, ensuring compliance with established care plans and maintaining continuity and quality of resident care.

Key Responsibilities

• Conduct comprehensive nursing assessments and formulate nursing diagnoses to guide individualized care plans.
• Develop, implement, and evaluate nursing care plans based on resident needs and physician orders.
• Coordinate and facilitate outside physician visits to ensure timely and appropriate medical care for residents.
• Administer routine and emergency medications and treatments, accurately charting all interventions and demonstrating thorough knowledge of medication actions and side effects.
• Initiate and manage IV therapy, including central lines and catheters, and administer initial IV medications.
• Ensure all clinical records reflect accurate, timely, and comprehensive documentation to support continuity of care.
• Provide effective communication and education to residents, families, and staff regarding care plans and health status.
• Promote and uphold the choices philosophy and resident rights in all aspects of care delivery.
• Implement and monitor safety and infection control procedures to maintain a secure and hygienic environment for residents and staff.
• Participate actively as a member of the interdisciplinary team in Quality Management programs to ensure high standards of care.
• Function as the primary contact person on the assigned shift, effectively supervising Nursing Assistants/Resident Assistants to ensure adherence to care plans.

Required Qualifications

• Current Registered Nurse (RN) license.
• Demonstrated experience in a Long Term Care (LTC) setting.
• Proficiency in nursing assessment, care planning, and medication administration.
• Ability to effectively supervise and delegate tasks to Nursing Assistants/Resident Assistants.
