Your Impactful Role as a Per Diem PTA

• Provide skilled physical therapy services in patients' homes by implementing treatment plans established by the supervising Physical Therapist, helping patients improve mobility, strength, balance, and independence.
• Monitor patient progress and document care accurately and promptly, communicating changes in condition and collaborating with the interdisciplinary care team to ensure high-quality patient outcomes.
• Educate patients on home exercise programs, safe mobility techniques, fall prevention, and the proper use of assistive devices to support continued recovery between visits.

Qualification & Skills

• Must hold valid MN PTA license
• Previous home health and/or outpatient experience strongly preferred
• Able to travel around the Twin Cities Metro area to provide services to client's (we pay mileage!)
• CPR/BLS Certification
