Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs across Arizona, Oklahoma, New Mexico, Oregon, and the District of Columbia. Founded in 1982, CBI’s mission is “maintaining the dignity of human life” and it provides residential, outpatient, inpatient, medication‑assisted treatment, crisis services, and patient‑centered medical homes for individuals with behavioral health challenges, substance use disorders, homelessness, and mental illness. Job Summary The Continuing Care Peer provides community‑based outreach, support, navigation, and care coordination for individuals experiencing behavioral health challenges, substance use, homelessness, or crisis situations. The role includes peer support, outreach, care planning, resource linkage onsite and in the community, and supports individuals during deflection encounters, sobering services, and transitions to appropriate levels of care, ensuring follow‑up, advocacy, and continuity of services. Skills / Requirements High school diploma or GED required. Minimum six months of recovery from substance use and/or mental health challenges. Experience in outreach, peer support, homeless services, or behavioral health preferred. Valid driver’s license required. Clear 39‑month motor vehicle record required. Peer Support Specialist certification from the Oregon Health Authority required or obtainable within 90 days of hire. Compensation & Benefits Hourly rate: $19.00–$22.80.
