Active PT license required (license required if applicable)
Can start with compact license but must obtain full state license during assignment
Experience in the outpatient setting required
One professional reference required
SSN required for consideration

Candidates with prior perm or PRN employment at this facility within the past year are not eligible to return until one year of separation has passed
