Job Summary Sign‑On Bonus: Up to $10,000 SOB based on eligibility. Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. Responsibilities Implements and monitors patient care plans; monitors, records and communicates patient condition as appropriate. Serves as a primary coordinator of all disciplines for well‑coordinated patient care. Notes and carries out physician and nursing orders. Assesses and coordinates patient’s discharge planning needs with members of the healthcare team. Assessment – a systematic, dynamic way to collect and analyze data about a patient, considering physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors. Initial assessment Re‑assessment Focused assessment based on patient condition, setting and population Problem identification – the nurse’s clinical judgment about the patient’s response to actual or potential health conditions or needs. Planning and outcomes – based on assessment and problem identification, the nurse sets measurable and achievable short‑ and long‑term goals in collaboration with the patient and family. Establishing daily goals with patient and family Formulating a plan of care Transition or discharge planning Implementation – nursing care is implemented according to the care plan, ensuring continuity of care for the patient during hospitalization and in preparation for discharge, and documented in patient’s record. Ensures patient safety Medication administration Provision of ordered treatmentsPatient/family education Documentation in health‑care record Evaluation – continuous evaluation of patient status and effectiveness of nursing care, modifying care plan as needed. Care coordination – actions initiated by nurses with patients, families, and/or members of their healthcare team to manage and correct the sequence, timing, and effectiveness of patient care from admission to discharge. Delegation Supervision Prioritization Qualifications Minimum Requirements: Education: Graduation from an accredited School of Nursing. Continuing education requirements as defined within the CARE program. Experience: One (1) year as an RN. Licensure/Certifications: Valid current Arizona Nursing License or compact state license and ability to become Arizona licensed within 30 days. Basic Life Support (BLS) American Heart Association and others as required by individual department as noted in Attachment A. Preferred Requirements: Education: Bachelor of Science in Nursing Licensure/Certifications: Advanced Cardiac Life Support (ACLS) American Heart Association Pediatric Advanced Life Support (PALS) Neonatal Resuscitation Program (NRP) or others as required by individual department.
