Overview
Licensed Practical Nurse, Long Term Care travel assignment in Rochester, New York. Start date 09/14/2026 for 17 weeks. Weekly pay range $983-$1,059 per week. Shift: 12-hour rotations (D/E/N). This on-unit role focuses on geriatric long-term care with wound care needs and no acute care requirements. You will provide direct patient care, including medication administration, and oversee CNA colleagues on a shift. Assignment offers rotating shifts and weekend/holiday on-call from unit needs; guaranteed hours are not guaranteed.

Responsibilities
- Deliver direct patient care, including medication administration, within the LPN scope of practice
- Monitor resident status, perform nursing assessments, and document care plans and progress notes
- Oversee CNA colleagues, coordinate tasks, and ensure care is delivered with dignity and safety
- Perform wound care per long-term care protocols, monitor for changes, and report concerns promptly
- Communicate effectively with the care team and escalate issues to supervising RN or physician as indicated
- Maintain accurate documentation, ensure infection control and safety standards, and participate in care planning and rounds

Requirements
- Current New York LPN license (active)
- BLS certification (American Heart Association)
- Minimum 2 years of experience in long-term care
- Ability to work in a geriatric long-term care environment, with rotating 12-hour shifts and weekend/holiday coverage as needed
- Demonstrated critical thinking, independence, and compassion in delivering resident-centered care
