SouthernCare is seeking a dedicated Hospice RN / RN Case Manager to deliver compassionate, patient-centered hospice and palliative care in patients' homes across Alabama. You will conduct comprehensive assessments, develop plans of care, educate families, and collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other professionals to ensure comfort and dignity at end of life.
