Job Summary

Aviata Health Group is seeking a qualified and compassionate Licensed Practical Nurse (LPN) to join our team at our Fort Myers, Florida facility. As a LPN, you will provide direct patient care to our residents and patients, ensuring they receive exceptional, person-centered care. About the Role

As a Licensed Practical Nurse (LPN), you will be responsible for providing direct patient care to our residents and patients, assisting them with activities of daily living, and implementing individualized treatment plans under the direction of a professional nurse. Your responsibilities may include:

• Assisting in the implementation of individualized treatment plans for each assigned resident

• Supervising CNAs and participating in their evaluation

• Overseeing Falls Program and Pressure Sore Program

• Providing nursing services

• Conducting and documenting a thorough evaluation of each resident's medical status upon admission and throughout their course of treatment

• Other duties as assigned

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN

• Experience in Skilled Nursing/Rehabilitation facilities preferred

• You must be qualified, compassionate, and dedicated to a job well done
