IOP/PHP Care Navigator

Universal Hospital Services Inc.

Responsibilities At St. Louis Behavioral Medicine Institute (SLBMI), the IOP/PHP Care Coordinator provides administrative support related to the success of all IOP/PHP programs, overseeing the admission process, clinical assignment, chart audit, and reporting to ensure accreditation standards for safe, high‑quality care. SLBMI is a freestanding outpatient facility serving patients locally in the St. Louis region and nationally via telehealth services. The position is primarily based at the Macklind location with 1–2 days per week assigned to the Chesterfield location. It is an in‑person role. Shift: Monday – Friday, day‑shift. Essential Job Duties/Responsibilities Oversee and improve patient entry, census, and discharge to regularly meet or exceed budgeted census expectations, including communication with referral sources and weekly reporting to senior leadership. Assist with initial and ongoing insurance authorizations for patients in IOP/PHP as needed. Maintain communication with patients from identification as an IOP/PHP patient through admission and discharge, answering pre‑admission questions. Monitor performance improvement and administer data tracking and reporting for IOP/PHP patients from initial evaluation to discharge, including symptom and satisfaction surveys. Communicate actively with treatment teams, attend program meetings, and track attendance via EHR records. Monitor and assist treatment teams during weekly staffing meetings to update treatment plans in alignment with compliance standards. Oversee documentation completeness, including treatment plans, quality improvement measures, discharge plans, and IOP‑related documentation; communicate deficiencies to the IOP Director and clinical staff. Coordinate with treatment teams to ensure implementation of discharge planning and completion of discharge summaries throughout the patient’s participation. Act as a liaison for outpatient treatment providers, facilitating internal and external referrals based on IOP/PHP after‑care and discharge plans. Attend all staff meetings relevant to IOP and the coordination of IOP patient care. Excellent communication and people skills. Good organizational and multi‑tasking abilities. Familiarity with office machines (e.g., fax, printer, scanner) and EMR use. Attention to detail and accuracy. Knowledge of spreadsheets and data manipulation and interpretation. Ability to work independently and implement time‑management skills. Customer service orientation. Problem‑solving skills. Ability to work in two locations throughout the week.
