This well-established health system is dedicated to providing holistic, patient-centered care through an expanding Palliative Care Program. The facility supports an interdisciplinary team environment that includes physicians, Advanced Practice Providers, social workers, and chaplains, all working collaboratively to deliver compassionate care to patients with serious illnesses in both inpatient and outpatient settings.

Job Details:

• Join a compassionate, interdisciplinary palliative care team dedicated to supporting patients and families during life's most meaningful moments
• Deliver holistic inpatient palliative care bedside consultation services alongside a Fellowship Trained Physician, APPs, social worker, and chaplain
• Provide care in both inpatient and outpatient/ambulatory palliative care settings
• Full-time schedule, Monday through Friday, promoting a healthy work-life balance
• Utilize EPIC Electronic Medical Record system for seamless documentation and care coordination
• Participate in the ongoing development and expansion of the palliative care program
• Significant experience in palliative medicine, oncology, hospice, or geriatrics is strongly preferred
• Board Certified or Board Eligible in Palliative Medicine required
