Physical Therapist (Full Time)

Responsibilities Physical therapist is responsible for providing physical therapy services and for adherence to all conditions outlined in the personnel services agreement. Assess and evaluate therapeutic, rehabilitative, functional status and participation in the development of the total plan of care; evaluate home environment. Assess muscle strength, mobility, gait, and range of motion to determine potential for rehabilitation. Direct physical therapy treatment. Initiate the physical therapy program and instruct other personnel, family members, and caregivers in the appropriate phases of care, explain program goals, and participate in case conferences. Train patients in the use of prosthetic devices. Provide physical therapy consultation to home families/caregivers when indicated and deliver in‑service education programs for nursing organization personnel as needed. When therapy is only a skilled service, instruct, supervise, and evaluate home health aide care every two weeks. Participate in discharge planning for patients and engage in peer consultation processes. Education and Work Experience Required Degree: Bachelor’s Degree in Physical Therapy from an accredited organization. Preferred Degree: Master’s Degree in Physical Therapy from an accredited organization. Certificates: PT License and CPR certification, if required by state. Experience: 2+ years of professional practice. Knowledge, Skills, and Ability Show compassion and understanding toward patients. Understand HIPAA laws and confidentiality policies. Create and implement treatment plans based on patients' specific conditions and needs. Develop customized treatment plans based on assessment findings. Strong observation, time‑management, detail orientation, communication, and interpersonal skills. Physical Requirements Stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard. Lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs. Operate various equipment as required by the department. Hear, understand, and distinguish speech and other sounds (e.g., machinery alarms, medical codes or alarms) in person, on the telephone, and via remote communication. Clarity of Vision Near visual acuity: clarity of vision at approximately 20 inches or less for tasks like reading small print or working with small objects. Far visual acuity: clarity of vision at 20 feet or more; ability to recognize features at a distance. Peripheral vision: ability to observe an area above and below or to the left or right while eyes remain fixed on a given point.
