We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team at Eastern Star Home in Oriskany, NY. As a key member of our healthcare team, you will have the opportunity to make a meaningful difference in the lives of our residents and their families.

Job Summary:

As an LPN at Eastern Star Home, you will provide high-quality patient care under the supervision of a licensed RN or MD. You will assist with various medical procedures, administer medications, and perform routine nursing tasks to ensure the health and well-being of our residents. Your excellent communication skills will also enable you to effectively collaborate with our interdisciplinary healthcare team to deliver exceptional care.

Responsibilities:

• Provide direct patient care under the supervision of a licensed RN or MD

• Administer medications and treatments as ordered by a physician

• Perform routine nursing tasks, such as vital sign checks, wound care, and catheter care

• Assist with activities of daily living (ADLs) and basic care needs

• Document patient care and medical information accurately and thoroughly

• Collaborate with the healthcare team to develop and implement individualized care plans

• Educate patients and their families on disease management, medication use, and other health-related topics

Requirements:

• Current LPN licensure in the state of New York

• High school diploma or equivalent required

• Minimum 0 years of experience as an LPN
