Physical Therapist - Home Health

As a Physical Therapist, you'll be responsible for planning and administering medically prescribed physical therapy treatments aimed at restoring function and relieving pain for patients with various injuries and disorders.

Key Responsibilities

• Plan and implement individualized physical therapy programs based on patient assessments and treatment goals.
• Provide education and information to patients and their caregivers regarding treatment plans and exercises.
• Participate in discharge planning and collaborate with the healthcare team to optimize patient outcomes.
• Complete all required documentation in compliance with policy and procedures.
• Direct and support patient participation in selected tasks to enhance performance and recovery.

Schedule

This position offers a full-time schedule of Monday through Friday, from 8:00 AM to 5:00 PM, with weekends required for orthopedic cases every 4th or 5th weekend.

Qualifications

• Minimum of 1 year of experience in physical therapy; first-time travelers are encouraged to apply.
• Experience in home health settings is preferred.
• Familiarity with MatrixCare or Epic is a plus, but not required.
• Valid Physical Therapy license for the state of Virginia is required.
• Current CPR certification is required.

Additional Information
