You'll work alongside an experienced interdisciplinary team dedicated to delivering exceptional hospice care.ScheduleFull-Time On-Call48-hour on-call weekend shiftFour weekday on-call shifts (Monday–Friday | 5:00 PM – 8:00 AM)What You'll DoRespond to after-hours patient needs and symptom managementPerform nursing assessments and develop appropriate interventionsProvide compassionate education and emotional support to patients and familiesCoordinate care with physicians and the interdisciplinary hospice teamComplete timely, accurate documentation in the EMRMaintain compliance with Medicare, state regulations, and hospice standardsParticipate in rotating on-call coverage while providing exceptional customer serviceWhat We're Looking ForCurrent Pennsylvania Registered Nurse (RN) License requiredMinimum of 1 year of RN experience (Hospice, Home Health, Oncology, ICU, ER, or Med-Surg experience preferred)Hospice experience preferred, but we're willing to train the right candidateStrong clinical assessment and critical thinking skillsExcellent communication and compassionate bedside mannerAbility to work independently while collaborating with the clinical teamValid driver's license, reliable transportation, and current auto insuranceWhy Nurses Choose VitalityAt Vitality Hospice, you're more than an employee—you become part of a team that genuinely cares about one another.
