Position Overview

We are seeking a licensed Physical Therapist (PT) to provide skilled therapy services to residents in accordance with physician orders and individualized care plans. You'll work closely with residents, families, nursing staff, and interdisciplinary teams to support recovery, function, and quality of life.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled therapy services in accordance with physician orders and care plans.
• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely.
• Collaborate with nursing and interdisciplinary teams to coordinate resident care.
• Participate in care planning and utilization review meetings.
• Educate residents, families, and staff regarding therapy goals, recommendations, and plans of care.
• Maintain established productivity and documentation standards.
• Support regulatory compliance and survey readiness.
• Promote resident safety, independence, and quality of life throughout the rehabilitation process.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, active Physical Therapist license in the state of practice.
• Long-term care or skilled nursing experience preferred.
• Strong clinical documentation and communication skills.
• Patient-focused, compassionate, and team-oriented.
• Ability to work effectively with residents, families, and interdisciplinary team members.

Physical Requirements

• Ability to stand, walk, bend, and assist residents during therapy sessions.
• Ability to lift, transfer, and support residents as required for safe treatment.
• Ability to function effectively in a clinical healthcare environment.
