Hospice Certified Nursing Assistant (CNA)

Wyoming Valley, PA | Full-Time

• Provide personal care to hospice patients in their homes and care facilities

• Assist with bathing, grooming, dressing, toileting, and other activities of daily living

• Provide repositioning, mobility assistance, and comfort-focused care

• Observe and promptly report changes in a patient’s condition to the assigned nurse

• Follow each patient’s individualized plan of care

• Maintain accurate and timely visit documentation

• Offer compassionate emotional support to patients and families

• Communicate effectively with nurses and other interdisciplinary team members

• Maintain patient dignity, privacy, safety, and comfort

• Current Pennsylvania Certified Nursing Assistant certification

• Hospice, home health, long-term care, or personal-care experience preferred

• Valid driver’s license, reliable transportation, and automobile insurance

• Ability to travel throughout the Wyoming Valley service area

• Compassionate, dependable, and comfortable working independently

• Strong communication and organizational skills

• Commitment to providing respectful and exceptional end-of-life care
