Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. • Works closely with healthcare providers to ensure that patients receive appropriate care and support services to improve health outcomes. • Conducts in-depth assessments of patient needs, goals, and barriers to achieving good health outcomes. • Provides advanced health education and coaching to individuals and families to promote healthy behaviors and self-management of chronic conditions. • Collaborates with healthcare providers to develop and implement care plans for patients with complex health needs. • Assists patients in navigating the healthcare system and accessing appropriate services.**Qualifications****Qualifications** * Bachelor's Degree Public Health, Social Work or other related field of study required. Related experience can be accepted in lieu of degree.* 2+ years of experience in community health outreach, health education, or related field required.* Community Health Worker (CHW) Massachusetts License preferred.* Experience with community-based work, patient home visits, SUD, homelessness and a complex patient population preferred.* Experience with Social Determinants of Health (SDOH), housing, food insecurity, transportation, utilities, employment, childcare assistance programs preferred.* Experience helping patients or clients apply for state/federal programs/benefits (i.e. SSDI, SSI, SNAP) and advocating for the patient with appeals preferred.* Knowledge of transportation resources (PT1) preferred. Additional Knowledge, Skills and Abilities: * Demonstrated ability to work effectively and provide advocacy for all populations and communities.* Strong communication and interpersonal skills, with the ability to interact effectively with various populations.* Ability to work independently and as part of a team.* Basic computer skills, including Microsoft Office and database management.* Bilingual skills preferred.**Additional Job Details (if applicable)****Schedule and Work Model:** * Full-time, Monday through Friday, standard business hours* Hybrid work model: Approximately 2 days/week in the community in Lawrence/Lowell area; approximately 2 days/week in the community in Peabody/Newburyport area; approximately 1 day/week remote working from home. Must be flexible for patient needs and team meetings as needed.* Includes patient in-home and/or physician visits.* On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment.
