Provide competent, safe, and compassionate care based on established standards. Formulate the initial plan of care with the provider and collaborate on a comprehensive patient-centered plan. Collect, assess, and document patient healthcare data. Involve patient/family and interdisciplinary team to identify a realistic and measurable plan of care. Prioritize care based on collected data. Document assessment and reassessment in medical records. Implement interventions in a safe, timely, and appropriate manner. Educate the patient/family regarding health status and established plan of care. Evaluate the patient's progress towards expected outcomes.
