Hospice Registered Nurse (RN) Position Summary
Safro Staffing is partnering with a hospice care organization seeking an experienced Hospice Registered Nurse (RN) to provide compassionate, patient-centered care to individuals and families facing life-limiting illnesses. The Hospice RN will coordinate care, perform comprehensive assessments, develop and update care plans, collaborate with interdisciplinary team members, and support patients and families throughout the hospice journey.

Safro Staffing is partnering with a hospice care organization seeking an experienced Hospice Registered Nurse (RN) to provide compassionate, patient-centered care to individuals and families facing life-limiting illnesses. The Hospice RN will coordinate care, perform comprehensive assessments, develop and update care plans, collaborate with interdisciplinary team members, and support patients and families throughout the hospice journey.
Responsibilities
- Coordinate hospice care services in collaboration with patients, families, physicians, and interdisciplinary team members
- Conduct comprehensive nursing assessments and hospice evaluations
- Develop, implement, and update individualized plans of care
- Monitor patient condition and respond appropriately to changes in clinical status
- Provide education and emotional support to patients and their families
- Coordinate care with contracted providers and community resources as needed
- Maintain accurate and timely clinical documentation in accordance with regulatory requirements
- Communicate effectively with attending physicians and the Hospice Medical Director regarding patient needs and care planning
- Participate in interdisciplinary team meetings, quality improvement initiatives, and staff development activities
- Support end-of-life care planning and symptom management to ensure patient comfort and dignity
- Current Registered Nurse (RN) license in good standing
- Previous hospice nursing experience required
- Strong clinical assessment and critical thinking skills
- Excellent communication, documentation, and interpersonal skills
- Ability to work independently while collaborating effectively within an interdisciplinary team
- Valid driver's license, reliable transportation, and current auto insurance
