Your Role as a Speech-Language Pathologist

As an SLP, you will provide skilled, individualized services to help residents achieve meaningful communication, cognitive, and swallowing goals while maximizing their independence and participation in daily life.
Key Responsibilities

• Evaluate referred residents to determine their speech, language, cognitive-communication, and swallowing needs.
• Assess medical condition, functional capabilities, limitations, and rehabilitation potential.
• Develop and implement individualized treatment plans based on each resident's needs, abilities, capacity, and tolerance.
• Establish realistic, measurable, and achievable therapeutic goals.
• Provide skilled speech-language pathology services in accordance with physician orders and established plans of care.
• Monitor resident progress and adjust treatment approaches as clinically appropriate.
• Educate residents, families, caregivers, and staff regarding therapy goals, treatment strategies, and recommendations.
• Complete accurate and timely evaluations, weekly progress notes, re-certifications, treatment documentation, and discharge summaries .
• Collaborate with nursing, physicians, rehabilitation staff, and other disciplines to provide coordinated, patient-centered care.
• Maintain confidentiality and comply with HIPAA, Federal Residents' Rights, and applicable regulatory requirements .
• Perform other duties as assigned by the Director of Rehabilitation.

Qualifications & Requirements

• Graduate of an accredited Speech-Language Pathology program .
• Current and valid Speech-Language Pathologist license in the state of Virginia.
• Strong clinical, communication, documentation, and organizational skills.
• Compassionate, patient-focused approach to resident care.
• Ability to work effectively as part of an interdisciplinary team.
• Long-term care and/or geriatric rehabilitation experience preferred , but not required.
