Medical Social Worker — Discharge & Care Coordination

100 Albany Med Health System is seeking a dedicated Social Worker to enhance patient outcomes by managing complex psychosocial needs in Albany, New York. You will work alongside treatment teams to develop Discharge Plans while advocating for patients' best interests and providing education on treatment options. The ideal candidate will possess a Master's degree in Social Work, have a valid NY State Licensure (LMSW), and have experience in Clinical Social Work.
