Care Coordinator

Supporting logistical patient care activities, the full-time Care Coordinator will manage remote duties including coordinating home-based care orders, collaborating with vendors, and ensuring patients receive necessary services while maintaining clear communication with the clinical team and other stakeholders. Key responsibilities Works with an interdisciplinary care team to assess and address complex care needs of patients and families Coordinates care by supporting admission and ongoing home-based census operations and following up on orders Provides administrative support for tracking ongoing care activities, including appointment scheduling and vendor liaison tasks Required qualifications Minimum one year of healthcare experience, preferably three years, with a focus on care coordination Associate's or Bachelor's degree in a health-related field is required Experience working with EMR and strong organizational skills in telephonic coordination Proficiency in Spanish is preferred Experience drafting organizational protocols and workflows is preferred
