Responsibilities
Provide direct patient care, including administering medications, performing vital signs, and assisting with personal care needs
Develop and implement individualized care plans, working closely with the interdisciplinary team to ensure the best possible outcomes
Observe and report changes in patient condition, communicating with the healthcare team to address any concerns
Participate in quality improvement initiatives, ensuring high-quality care is consistently delivered
