Flexible PRN scheduling that fits your lifestyle Join a team that's growing to support quality care-not simply replace vacancies Focus on meaningful patient relationships in a one-on-one setting Supportive clinical leadership and collaborative interdisciplinary team Competitive pay Opportunity to make a lasting impact in your local community National leader in home health with a patient-first culture As a PRN Home Health RN, You Will: * Deliver skilled nursing care to patients in their homes while promoting safety, independence, and quality of life. * Complete comprehensive nursing assessments and develop individualized plans of care. * Perform admission, routine, and follow-up visits based on physician orders and patient needs. * Monitor patient progress and identify changes in condition, communicating findings promptly with physicians and the interdisciplinary care team. * Educate patients and caregivers on disease management, medications, treatments, and self-care. * Coordinate care with therapists, aides, physicians, and other healthcare professionals to ensure seamless patient care. * Accurately complete clinical documentation in a timely manner. * Maintain compliance with agency policies, HIPAA regulations, Medicare Conditions of Participation, and professional nursing standards. * Deliver compassionate, patient-centered care that aligns with Elara Caring's mission and values. What You'll Need * Current, unrestricted Registered Nurse (RN) license in the state of practice. * Minimum of one (1) year of nursing experience in an acute care, skilled nursing, or clinical care setting. * Previous home health experience is preferred, but not required. * Experience with HomeCare HomeBase (HCHB) is a plus. * Strong assessment, communication, and critical thinking skills. * Reliable transportation, valid driver's license, and current automobile insurance. * Ability and willingness to travel throughout the assigned service area. * Physical Requirements: Sit, stand, bend, lift, and move intermittently and be able to lift 50-100 lbs.
