LPN - Continuing Care

Location: Grand Rapids, MI Duration: 13 Weeks (Sep 8, 2026 - Dec 7, 2026) Shifts: Nights, 3x12, 6:30 PM - 7:00 AM, 36 hours per week

Job Description

• Provide direct patient care under the supervision of an RN, Mid-Level Provider, or Physician.
• Assess, plan, implement, and evaluate patient care for residents in a continuing care setting.
• Document patient care accurately in the electronic medical record system.
• Educate patients and families regarding treatments, medications, and care plans.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care.
• Monitor and report changes in patient condition, including physiological and psychosocial needs.
• Maintain a safe and clean patient environment.
• Participate in quality improvement and patient safety initiatives.

Requirements

• Active Michigan LPN License.
• BLS Certification (AHA or ARC).
• Previous Rehab/SNF experience required.
• EPIC experience required.
• Experience with tracheostomy care.
• Experience with patient admissions.
• Experience with feeding tubes.
• Experience caring for neuro patients.
• Ability to work in a fast-paced environment and complete tasks efficiently.
• Every other weekend availability.
