ER Case Manager: Fast-Paced Discharge & Care

UMass Memorial Health in Milford, MA is seeking a Point of Entry/Emergency Department Case Manager (POE/ED CM) to coordinate care for ED patients, evaluate admissions, and determine discharge needs using InterQual criteria. You will collaborate with MSW, CM, and ED teams, arrange post-acute services, and communicate with families.
