Job Title: Home Health Registered Nurse Job Responsibilities Conduct initial assessments and develop individualized care plans for clients Provide skilled nursing care to clients in their homes, including medication management, wound care, and symptom management Coordinate with other members of the care team to ensure comprehensive and integrated care for clients Educate clients and their families on proper self-care techniques and health maintenance strategies Monitor and document client progress and communicate any changes or concerns to the physician Act as an advocate for clients and ensure their needs are met in a timely and respectful manner Adhere to all company policies, procedures, and regulatory requirements Job Qualifications Active RN license in the state of Nebraska Minimum of 2 years of nursing experience, preferably in home health or a similar setting Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Ability to work independently and collaboratively as part of a team Compassionate and patient-centered approach to care Company Overview Freedom in Home Services is committed to providing exceptional care and support to individuals in the Omaha community.
