Current licensure and Certificate of Authority to practice as a Physician Assistant in the state of Ohio required. DOT Cert. and X-Waiver preferred.For Nurse Practitioners:Master's degree in Nursing and completion of an accredited Nurse Practitioner Program; Drug Enforcement Administration (DEA) Registration/Certification; Current licensure and Certificate of Authority to practice as a nurse practitioner in the state of Ohio; FNP certification preferred.In addition to the above qualifications, all candidates should meet the following:BLS & ACLS certification2+ years' experience in family practice environment preferredExcellent communication skills and customer service skillsAbility to work independently, but also able to collaborate effectively with other staff members to provide quality patient careBehavioral health experience desirableMaintains required training in accordance with the Mainstreaming Addiction Treatment Act (MAT Act)
