Physical Therapist

Role: Licensed Physical Therapist providing personalized care to older adults in private homes or senior living communities, primarily through the Geriatric House Calls model. Responsibilities Offer one-on-one physical therapy, including evaluations, treatment planning, and evidence-based therapy sessions. Support patient mobility, independence, and wellness through tailored interventions. Document care accurately in the EMR system, adhere to regulatory standards. Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Incorporate telehealth and virtual care to enhance access and flexibility. Qualifications & Skills Valid Physical Therapy license or eligibility in practice state(s). Degree from an accredited PT program. Proficiency in basic computer skills and EMR systems. Current CPR certification. Key Duties & Requirements Meet physical demands of in-home and community care, including lifting, repositioning, and assisting patients. Drive to patient locations, wear PPE as needed, and perform CPR if necessary. Support patient transfers, standing/walking, and therapeutic activities.
