Registered Nurse Case Manager

Avera Health

Avera Health is seeking an experienced Registered Nurse to join the Avera at Home Mitchell team for comprehensive in-home patient care. You will assess, plan, and coordinate nursing services in collaboration with physicians and the interdisciplinary team to achieve optimal patient outcomes. You will educate patients and families, perform Start of Care visits, re-certifications, and supervisory visits, and ensure compliance with care standards and safety guidelines. #J-18808-Ljbffr
