Chaplain (PRN)

Provides spiritual care to patients, families, and staff at hospital as appropriate and complete spiritual assessments. Collaborates with care teams, volunteers and faith community to integrate physical, emotional and spiritual healing for the patient. Communicates with local ministers and priests as needed, giving pertinent information regarding care of patients and other necessary hospital information. Maintains timely documentation of spiritual care services, which reflect the pertinent information for spiritual assessment, spiritual care involvement, outcomes and recommendations for plan of care. Responds to referrals from staff, patients, and families and well as being responsive to crisis intervention, medical and trauma alerts. Assists with organizing volunteers to distribute Holy Communion to Catholic patients and residents at scheduled times. Fills in for volunteers in their absence as available. Participates in the collaboration with local and retired priests to ensure mass coverage for St. Anthony residents. Attends Eucharistic celebration as available; assists residents to mass and devotions when needed. Leads morning prayers. Shares on-call coverage with Spiritual Care staff and volunteers. Participates in creating seasonal displays throughout campus reflecting different ‘seasons of the church’. Participates in memorial/prayer services for staff as needed. Helps coordinates ‘No One Dies’ Alone Program, and Food Drives throughout the year. Participates in educating Administration and staff on Franciscan Legacy and Values. Exhibits a presence at Hospital & Nursing Home events. Shares on-call coverage with Spiritual Care staff and volunteers. Participates in ongoing, formal evaluation of practice to assess quality and appropriateness of services, to improve practice, and to ensure competence. Effectively utilizes supervision to improve practice. Demonstrates age specific competence, including knowledge, behaviors and community resources, to meet patients’ needs of all age groups, neonates to geriatric populations; work collaboratively with age-specific hospital-based resources, including Geriatric specialists, pre- and post-natal specialists, and other clinical specialists. Provides leadership to ensure access to care, to improve and maintain the quality of care, demonstrated by participation in teams and work groups. Mentors others within and outside the spiritual care profession. Shares on-call coverage with Spiritual Care staff and volunteers. Oversees, develops, and helps implement the Quality Assurance and Performance Improvement (QAPI) for the assigned departments. Practices and behaves in a professional, safe and ethical manner at all times. Serves as a patient advocate. Maintains friendly and courteous interpersonal communication skills and a positive service attitude. Participates in performance improvement and educational activities. Completes all CEU’s as required by respective licensure. If licensed, maintains an active and current license. Works willingly and accepts assignments as given. Attends all mandatory meetings and programs. Participates in opportunities that enhance personal and professional growth. Performs other duties as necessary or assigned. Maintains the confidentiality of patient, resident, employee and organizational information. Travel Required No. Education Required Bachelors or better. Experience Required 1-2 years: Chaplain experience preferred.
