Day one includes in-person training at our West Seneca, NY office to introduce our culture and Care Coordination Hybrid Schedule Care Coordinators are required to attend three onsite days per week during the first 90 days to support onboarding and integration After successfully completing this period, team members may transition to a hybrid schedule for added flexibility. Supervisors may extend the on-site requirement if needed. Position Overview Person Centered Services Care Coordination Organization is looking for passionate Care Coordinators to work within a health home model serving people with intellectual and developmental disabilities. The Care Coordinator has an overall responsibility for coordinating and advocating for all aspects of the individual’s care including health and behavioral healthcare, community supports, and other services required to meet the needs of the individual. Care Coordinators use a person-centered, holistic approach, where each person being served has their health care and other service providers collaborate to ensure their wants and needs are met. We’re in the business of helping people and always using a strengths-based approach. Social workers, care managers, and case managers are encouraged to apply! Position Responsibilities Develop a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team. Commit to a respectful, just, and supportive environment for individuals and coworkers aligning with the company’s commitment to diversity, equity, and inclusion. Effectively manage a tiered caseload, while tailoring services to individual needs. Complete enrollment and eligibility documentation. Meet with individuals in their homes, physician/provider offices, and other public places in order to conduct assessments and provide services. Work independently from home, as well as report to the office at least once per week but may require more depending on business needs. Requirements Educational or licensure qualifications: Bachelor's degree with 2 years of relevant experience or a Licensed Registered Nurse with 2 years relevant experience or Master's degree with 1 year of relevant experience Relevant experience consists of providing direct care or support, coordination, and/or advocacy for any person(s) on a regular basis.
