Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 260 days Shift: 2x3 days Hours per Shift: Not specified Experience: Minimum of two (2) years in a home health setting within the last fifteen (15) years License: Current RN Licensure in the state Certifications: BLS required before the start date Must-Have: Bilingual proficiency in English and Spanish strongly preferred Recent experience conducting comprehensive assessments of patients Description: We have an urgent need for a registered nurse specializing in home health. The role requires the ability to conduct initial in-home evaluations of patients, assessing health status, strengths, care needs, and preferences. This assessment guides the development of individualized long-term care service plans. Candidates must review consumer medical documentation and relevant health referral forms. Entry of evaluation data into an electronic form and timely transmission is required. Documentation of any concerns or conflicting information during the evaluation process is essential. Continuity of care must be emphasized to reduce gaps in treatment plans. The nurse will discuss healthcare options and supports with consumers. Communication with eligible consumers for enrollment into a managed care health plan is part of the role. Enhancing communication with the interdisciplinary care team to improve coordination of care is crucial. The collection of quality review data to support outcome measurements and documentation of case notes is necessary. Opportunities for health promotion and illness prevention should be identified. A comprehensive knowledge of community resources and service network for the target population is required. All standards established for this position must be met as outlined in the annual performance criteria. Other duties may be assigned by management as needed.
