LPN, Long Term Care

Provides nursing care services consistent with responsibilities of LPN. Observes and reports symptoms and conditions of residents. Administers medications as prescribed by physician. Gives treatments as ordered. Counts narcotics and maintains record of same. Maintains records reflecting resident condition, medication, treatments and progress. Transcribes physician-written and telephone orders. Places drug orders. Assists with completion of nursing history and assessment, MDS, and care plans. Assists in orientation and supervision of nursing staff. Performs direct resident care as necessary. Conforms to recognized nursing practice and facility policies and procedures. Participates in ongoing in-service education. Assumes Charge Nurse Responsibilities when necessary. Prepares/coordinates CNA assignments. Responsible for calling MDs, booking tests, labs, x-rays, and medical appointments. Assists the MDs in making rounds and preparing charts for MD signatures. Checks meal tray for accuracy. Directly supervises CNAs. Performs all treatments and assessments as required. Handles resident admissions and discharges. Notifies MD/NP of any changes in resident status. Informs family members/HCP of changes in treatment status. Responsible for documentation/nurses' notes. Updates care plans as necessary. Answers to nurse managers. Physical Requirements: Able to lift up to fifty pounds. Able to push/pull forty pounds. Able to stand for up to four hours per day. Able to bend, squat, and stoop to floor level. Able to kneel. Able to walk. Able to reach heights of up to four and one-half feet. Able to see and hear with or without prostheses. Able to move intermittently. Able to cope with physical and mental job stress. Required Knowledge, Skills and Abilities: Ability to recognize changes in the condition of residents and to take action required.
