RN Registered Nurse

To support members with chronic and complex health conditions, the seasonal, fully remote Senior Care Specialist (Registered Nurse) will conduct telephonic health assessments, develop individualized care plans, and coordinate services to promote improved health outcomes. Key responsibilities Conduct telephonic health assessments for members and develop actionable, individualized care plans Establish health coaching relationships and provide education and support for chronic condition management Collaborate with multidisciplinary teams to coordinate care and address barriers to health Required qualifications Active, unrestricted Registered Nurse (RN) license in the state of residence Minimum of 2 years of clinical nursing experience; 3 or more years preferred Strong clinical judgment and ability to work independently in a remote environment Intermediate to advanced computer proficiency, including Microsoft Office applications Ability to efficiently navigate multiple computer systems and electronic medical records
