Licensed Vocational Nurse (LVN) - Home Health

Location: Arlington Service Area
Schedule: Monday-Friday, 8:00 AM-4:30 PM
Hours: 40 hours per week (8 hours per day)
On-Call/Weekend Requirement: Yes
Productivity Expectation: 30 points per week
Mileage Reimbursement: Yes, between patient visits

Position Overview

We are seeking an experienced and compassionate Licensed Vocational Nurse (LVN) to join our Home Health team serving the Arlington area. The LVN will provide skilled nursing services to patients in their homes, helping them maintain their independence and improve their quality of life. This role requires strong clinical skills, excellent communication, and the ability to work independently in the field.

Key Responsibilities

Perform skilled nursing visits in patients' homes in accordance with physician orders and the established plan of care.
Assess and monitor patient conditions, reporting significant changes to the clinical team.
Provide specialized treatments and procedures
Educate patients and caregivers on disease management, medications, treatments, and safety measures.
Accurately document patient assessments, treatments, and progress notes in accordance with agency standards.
Collaborate with physicians, RNs, therapists, and other healthcare professionals to coordinate patient care.
Participate in on-call and weekend coverage as assigned.
Meet productivity expectations while maintaining high-quality patient care.

Required Qualifications

Current and unrestricted Licensed Vocational Nurse (LVN) license.
Current BLS (Basic Life Support) certification.
Previous home health experience .
