RN Case Manager
Good Samaritan
Good Samaritan is seeking an RN Case Manager to coordinate whole-person care for underserved patients in our faith-based health center. In this role, you will manage complex care plans, perform clinical and psychosocial assessments, support care transitions, and connect patients with community resources. You’ll collaborate with an interdisciplinary team to advance health equity, improve outcomes, and reduce barriers to care, all while practicing in a mission-driven, compassionate, and team-oriented environment in Atlanta.
