Duties Description This LPN is needed to provide hands on nursing coverage for the individuals with varying medical needs who reside in Meadow Lane IRA and Rudd Pond IRA. Meadow Lane IRA will serve as the primary location for this position and accommodates five individuals with varied medical and behavioral requirements. The Rudd Pond IRA provides care for four individuals with different nursing needs, including tasks that cannot be delegated to unlicensed direct care staff. Key responsibilities will include developing patient care protocols and training staff on necessary care procedures. The LPN will ensure compliance with patient care standards and provide services in a timely and appropriate manner, according to individualized treatment plans. Additionally, the LPN will evaluate patients’ responses to treatments and medications, as well as any changes in their health status, and will promptly communicate these observations to a Nurse II or physician. The LPN will address concerns raised by patients, families, and staff, escalating issues to higher-level personnel when necessary. Maintaining accurate documentation of patients’ conditions and the treatments administered is also a critical aspect of this role. The LPN may be asked to provide coverage at other sites within the team if care is needed. Nursing oversite will be provided by the Registered Nurse 1, with administrative oversite provided by the Registered Nurse Supervisor 1. Duties include, but are not limited to:• Assist with nursing tasks within their scope of practice• Assist with the individuals’ personal hygiene and daily living tasks• Ensure that all medications and treatments required on that shift are performed and implemented as written, and the data is recorded.• Provides documentation as needed • Ensures that the following documentation is accurate and available such as The Ready to Go, monthly MAR, Consents, Current Scripts• Maintains tickler for OTC expiration dates and OTC orders for the individuals• Monitors/supervises individuals with insulin administration, and with PO daily medications/treatments• Provides first aide to individuals and monitors vital signs as needed and reporting to the Registered Nurse 1 their findings• Acts as liaison between the IRA Residential staff and the Registered Nurse 1.• Purchasing and organizing med room supplies • Other duties as assignedNOTE: You must have a valid driver’s license to apply as travel is a part of the duties involved in this position. Minimum Qualifications Either A: A license and current registration to practice as a practical nurse in New York State; OR B: A limited permit to practice as a practical nurse in New York State; OR C: An application on file for a limited permit to practice as a practical nurse in New York State. (You must be approved immediately following graduation from an approved school of practical nursing in New York State to qualify under this option.) Note: If you are appointed with either qualification "B" or "C," you must obtain your license within one year of the date your limited permit was issued, or steps will be taken to remove you from the position. Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.• Please note that the posted Hiring Salary includes the sum of the statutory hiring rate for CSEA Grade 10 ($45,081), along with any special salary treatments in effect for this position in this location, such as a Geographic Pay Differential ($13,000), Increased Hiring Rate ($5,852) and Mid-Hudson adjustment ($2,000). Geographic Pay Differential and Mid-Hudson adjustment are also added to the job rate ($55,322).• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.• For CSEA positions: Local Bidding Agreements will be honored.• Preference will be given to OPWDD employees impacted by closures.
