Location: Northern Dauphin Nursing and Rehab | Millersburg, PA
Schedule: Full-Time | Monday–Friday

Position Overview

Northern Dauphin Nursing and Rehab is seeking a compassionate and experienced Social Services Director to lead the Social Services Department at our skilled nursing facility in Millersburg, PA. This leadership role serves as an advocate for residents' social and emotional well-being while overseeing all aspects of social services programming, regulatory compliance, and departmental operations.

The Social Services Director supports residents, families, and staff through periods of transition, adjustment, and crisis while ensuring compliance with federal, state, and facility regulations and promoting exceptional resident-centered care.

Key Responsibilities

• Complete comprehensive social assessments and develop individualized care plans that address residents' medically related social and emotional needs

• Participate in pre-admission and admission processes to ensure smooth resident transitions

• Serve as a compassionate liaison for residents and families, addressing concerns and providing ongoing support

• Maintain accurate documentation of residents' behavioral, emotional, and psychosocial status in accordance with regulatory requirements

• Provide individual and group counseling and support during periods of adjustment, crisis, or special circumstances

• Develop, coordinate, and maintain a volunteer program to enhance resident quality of life

• Orient new residents and families to the facility and assist with successful adjustment to the community

• Collaborate with the interdisciplinary team to promote resident well-being and achieve positive outcomes

• Ensure compliance with all federal, state, and facility social services regulations and standards

Qualifications

• Bachelor's Degree in Social Work or a related field from an accredited institution required

• Previous experience in long-term care or skilled nursing preferred

• Thorough knowledge of social service practices, regulations, and guidelines governing long-term care

• Excellent communication, documentation, organizational, and interpersonal skills

• Ability to work collaboratively with residents, families, and the interdisciplinary care team
