Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Conduct 60-minute evaluations and 30-minute follow-up sessions; delegate and direct discipline-specific therapy services for assigned patients
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all regulatory requirements; maintain active professional licensure in good standing and complete all state-mandated continuing education requirements

Required Qualifications

• Active Physical Therapist license in the state of Colorado
• Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in medical record documentation in compliance with state, federal, and intermediary guidelines
• Commitment to patient and employee safety standards and adherence to professional codes of conduct

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring physical therapy students
• Familiarity with performance improvement methodologies and quality initiatives
