MDS Registered Nurse Coordinator

This role is responsible for overseeing the development and completion of the Resident Assessment Instrument (RAI) through accurate and timely completion of the Minimum Data Set (MDS), in compliance with state and federal regulations and facility policies. The MDS RN Coordinator is a key member of the interdisciplinary team, ensuring quality care, continuity of services, and patient-centered care planning.

Key Responsibilities:

• Conduct patient assessments to determine health status, level of care, and any changes
• Ensure accurate and timely completion of RAI/MDS
• Coordinate interdisciplinary team involvement in care planning
• Support initiatives to improve care quality and maximize reimbursement

Requirements:

• Active RN license in Pennsylvania (PA)
• Experience as an MDS Coordinator
• Hands-on experience in MDS completion
