We are seeking a skilled and compassionate ED Nurse, who understands and respects the values of our tribal communities. Together, staff will provide care that meets the needs of the community and offers a meaningful and culturally responsive experience for both patients and their families. Job Summary Provides complex or clinical specialty area care for people living with diabetes in accordance with established protocols and multi-disciplinary plan of care utilizing independent and interdependent interventions to restore stability, prevent complications, and achieve and maintain optimal patient responses through patient-centered / patient-valued care. Job Duties Coordinate diabetes prevention and education program activities, resources, referrals, enrollment, and participant support fordiabetes programservices. Build and maintain relationships with clients, providers, partner organizations, program sites, and community resources to support program access and outcomes. Provide technical assistance to program sites, including site visits, data collection, marketing support, and coordination of best practices. Track, organize, and report program data related to referrals, enrollment, participation, completion, and ongoing program needs Develop and support program communications, educational materials, newsletters, webinars, fact sheets, and outreach resources. Stay informed on program updates, industry trends, company initiatives, and public health best practices Follows Tribal Health and client specific policies and procedures. All other duties as assigned and specified by clinical assignment and site.
