We are seeking a Home Health Physical Therapist or Assistant to provide skilled care for post-surgical patients, older adults, and individuals with neurological conditions. Reporting to the Clinical Operations Manager, this role delivers care through fully scheduled home visits, flexible route-based visits, and mixed home and facility visits.

Responsibilities Assess patients and develop individualized care plans. Conduct home and facility-based patient visits. Provide therapeutic exercises, mobility training, and gait training. Perform manual therapy and gait analysis as appropriate. Educate patients and coordinate care with relevant providers. Document clinical services accurately in electronic health records.

Required Qualifications State physical therapy or assistant license. Current CPR certification. Spanish Speaking

Preferred Qualifications Entry-level experience in physical therapy. Bachelor’s or Master’s degree in Physical Therapy. Knowledge of patient assessment, therapeutic exercise, gait analysis, and manual therapy. Experience with electronic health records and clinical documentation. Skills in patient education, care coordination, and communication.
