Senior Care Coordination Social Worker

Wellstar Health System is seeking a Care Coordination Social Worker Sr. to assess transitional care needs, coordinate across the continuum, and support patients and families in discharge planning. You will serve as a consultant on psychosocial factors and connect families with community resources to enable safe transitions. In collaboration with the care team, you will mentor peers, participate in interdisciplinary rounds, and ensure documentation in the EHR reflects the care plan and progress. #J-18808-Ljbffr
